A Referral →

A student's journey starts with a referral to Farm Club. We accept referrals for students with a wide range of needs and from varying backgrounds, however we cannot take direct referrals from parents/carers.

Here’s a quick guide through the referral process:

 
 
  • Enquiry is made by referrer via website, call or email.

  • Following an enquiry, the referrer will receive an email from Farm Club explaining the provision that’s available alongside next steps.

  • If there are further questions at this stage, the referrer should respond requesting a phone call with a member of the team at farm club, prior to proceeding to step 4.

  • The referral form should be completed and an EHCP / character report sent via email to Farm Club at the same time. The referral form can be accessed via direct link in an email from Farm Club, or on the Farm Club website.

  • Upon submitting a referral form and EHCP / character report, the referrer should await communication from Farm Club - our team requires this time to read over all referral documentation and assess the EHCP / character report and session availability.

  • Following a successful referral, an email from Farm Club will be sent including induction date and time, proposed start date, terms of business, induction invoice and LEJ form

  • The referrer must respond acknowledging induction timing, provisional start date, signed terms of business, processing induction invoice, completed LEJ form - all of these documents must be received / completed prior to an induction taking placeItem description

  • The student, parent/carer and a representative of the school/council should attend for this session.

    Important information: An induction session is an opportunity for Farm Club to assess the needs of each provisional student and see how they respond in the space as well as the other children that attend the setting. It is imperative to note that Farm Club are not responsible for the prospective student whilst they are visiting for the induction session. Parents/carers and referrers must be mindful of this fact and only attend the induction session with the understanding that they are responsible for the behaviours and welfare of the child. The induction session is also the first real opportunity that Farm Club staff get to assess whether we are able to meet the needs of the student. Following an induction session, there is still a possibility that the provision cannot go ahead. It is therefore vital that students are not told they will be attending Farm Club in the future, so as not to invoke any feelings of rejection if the placement doesn’t continue. We like to proposition these sessions to the student as a ‘look-around’ or a ‘tour’.

  • An assessment of the induction will be made by Farm Club staff and fed-back to the referrer, including all final documentation and guidance for the process - we practice honesty and professionalism when assessing whether we are able to meet the needs of each student.

  • If the induction is successful, carer permissions, transport passport and the service level agreement should be signed and returned. All relevant invoices must also be processed in order to proceed. Continue to Step 11.

  • At this point, the referrer will be informed that a place can not be granted and given sufficient explanation as to why. The process ends here.

  • The provisional start date will be confirmed at this stage, once all documentation has been received and is completed to a satisfactory standard.
