Please use the below form if you’re looking to get in touch with us here at Farm Club. This can be to ask further questions, determine whether the student you’re looking to refer is suitable for the provision or simply to ask for some advice. If you are a parent/carer/responsible party for the child, this is a great way for you to get in touch before speaking with your referring agency and encouraging them to move the process along.

Please click the below link if you’re ready to make a referral to Farm Club for a student within your care. This form is for the use of professionals. We do not accept direct referrals from parents/carers. It is important that this form is NOT completed until funding for the student is in the process of being agreed (as a minimum), provision has been discussed with parents/carers and until you’re ready for an induction imminently. If you wish to be added to a waitlist because these steps have not yet been agreed, please use the enquiry form on this page for further advice.

IT IS GOOD PRACTICE TO SUBMIT AN ENQUIRY AHEAD OF SUBMITTING THE REFERRAL FORM, SO YOU CAN ENSURE YOU’RE FOLLOWING THE CORRECT PROCESS.
